A woman in Oklahoma accused of practicing with bogus state dental hygienist licenses for more than four years has been charged, according to an affidavit of probable cause from the District Court of Canadian County for the State of Oklahoma.
Christy Perkowski, 46, was arrested and charged with five counts of practicing dental hygiene without a license and two counts of uttering a forged instrument. If convicted on all counts, she faces more than one year in prison plus fines.
In April, the Oklahoma Board of Dentistry was notified that Perkowski allegedly used a forged license to work as a dental hygienist at a dental clinic in Yukon, OK, according to the affidavit.
Between January 2001 and April 2025, Perkowski allegedly worked on patients, including providing preventive treatments and administering local anesthesia and nitrous oxide. Her dental hygienist license expired in 2020.
To continue working at the dental clinic, Perkowski allegedly presented forged 2022 and 2024 Oklahoma Board of Dentistry registered dental hygienist licenses to staff members, according to the affidavit.
A criminal complaint is merely an allegation. All defendants are presumed innocent until proven guilty beyond a reasonable doubt in a court of law.
