The phrase “digital co-worker” sounds a little like something the artificial intelligence industry would invent. It conjures the same big questions that have followed AI into nearly every profession: How capable will it become? What will it do next? How dramatically could it change the way a business operates?
But inside a dental practice, the more interesting question may be much smaller. What if the real value of AI is simply making sure the ordinary work gets done every single time?
That distinction matters, because most dental offices are not short on software. They are short on time, attention, and help. The phone rings while someone is checking in a patient. Tomorrow’s appointments need to be confirmed. A patient who left without scheduling their next hygiene visit needs a follow-up. Insurance still needs to be verified. A handful of patients became overdue for recare today, and the recare list continues to grow. Someone on the website has a question.
None of those tasks is unusual. But each one matters, and each one is easy to lose track of when the day gets busy. That is precisely the problem.
From raising the ceiling to raising the floor

AI adoption in dentistry is already well underway. In a 2026 survey from the ADA’s Health Policy Institute, 43.3% of responding dentists said they were using AI for at least one task in their practices.
Much of the conversation around that adoption focuses on what AI can add: more revenue, more production, faster growth, new capabilities.
There is another way to think about it. Instead of asking how much higher AI can raise the ceiling of a dental practice, ask whether it can raise the floor.
Can every patient who becomes overdue receive follow-up?
Can every appointment receive the appropriate confirmation?
Can every insured patient get a first-pass benefits check?
Can the practice respond to a website chat even if only a handful arrive each month?
Can a patient who leaves without scheduling their next appointment automatically enter the right follow-up process?
Annie, an AI-powered platform for dental practices, was one of the first AI receptionists built specifically for dentistry, initially focused on answering calls, responding to patients, and scheduling appointments.
Now, the company is launching what it calls a “digital co-worker.” The bigger idea is not simply what the software can do but how it operates inside the practice.
Annie rebuilt its software around a different model: Instead of waiting for the team to open a tool, work a list, or start a task, it continuously watches for work that needs to happen, proactively completes tasks, and then brings the team in when something requires more judgment, just like a co-worker or teammate would do.
That model now extends into recare, reappointment, insurance verification, appointment confirmation, and other day-to-day work across the practice. It is a shift from software as a tool to software acting more like a sidekick, and it makes Annie an interesting case study for what the next phase of AI in dentistry could look like.
Recare is not really a list
Consider recare. Most practices are familiar with the idea of “working the recare list.” The phrase itself reveals how the process usually works. Patients accumulate on a list, someone finds a block of time, and the team begins making calls.
During a busy week, the list can wait. A digital co-worker changes the unit of work from the list to the patient.
When a patient crosses the practice’s threshold for being overdue, that patient can enter an outreach sequence. Tomorrow, another few patients might become overdue. The system does not need a dedicated afternoon to call 300 people. It can work the problem continuously, a few patients at a time.
Annie says its recare calls currently produce about a 20% connect rate through its digital co-worker compared with an average of 25% when a human calls. A 20% connect rate is not a bombshell number in isolation. The bigger story is the baseline it creates: Every patient who becomes overdue gets contacted right away, without waiting for someone to work a recare list. Follow-up becomes continuous rather than periodic, making it much harder for patients to simply slip through the cracks.
The evidence matches that narrative. According to company data, across practices that have allowed their Annie digital co-worker to help with recare, those co-workers produce an average of 13.4 scheduled appointments per office per month, representing roughly $6,600 in direct monthly production, before accounting for any subsequent restorative or other treatment diagnosed as a result of bringing those patients back into the practice.
Recare also illustrates what Annie sees as the difference between a digital co-worker and traditional software. Software typically waits for someone to open it, check a list, or tell it what to do. A co-worker can keep an eye on what is happening and notice when something needs attention. In that sense, Annie is designed to “babysit” each patient’s journey through the practice, watching for small gaps and triggering follow-up when they appear.
And that can start before a patient ever becomes overdue. Henry Schein One’s analysis of more than 2,500 dental practices found that the average practice reappointed 77% of patients following a completed visit. In other words, nearly 1 in 4 completed appointments did not result in a future appointment being scheduled at that time.
If someone leaves the office without scheduling their next visit, an Annie digital co-worker can notice that too, follow up, and keep the patient moving forward without waiting for someone on the team to remember. That is the shift AI is bringing to software: from tools people operate to systems that can notice, act, and follow through.
Insurance verification shows why a baseline matters
Insurance verification may be an even clearer example. The ADA recommends verifying eligibility on the date of service, noting that offices typically do so through a payer portal or by contacting the insurer directly.
The potential volume adds up quickly. Henry Schein One’s benchmark data put the average dental practice at roughly 36 scheduled appointments per day. Using 20 operating days as a simple monthly model, that is approximately 720 appointments per month.
Using those figures as a directional model, we can estimate that an average volume practice could encounter roughly 550 insurance verification opportunities in a month. Industry estimates commonly put a manual verification at roughly 10 to 15 minutes when staff must navigate portals, gather benefits, or contact a payer.
If a digital co-worker can take a meaningful chunk of the routine verifications off the team’s plate, that alone can save hours of work. At even the low end of that range of 550 checks, Annie estimates that having its co-worker help with verifications saves its practices more than 90 hours of potential manual verification work per month, if not more. This then frees up the in-office team’s time to solve the truly difficult insurance cases.
What about my automated text messages?
Appointment confirmations illustrate the same shift. Automated reminders have existed for years. A practice management system can text a patient, “Your appointment is tomorrow at 2 p.m.”
The interesting part begins when the patient replies, “I can’t make it.” At that point, the reminder software has technically done its job while the practice’s work is just beginning.
Someone must respond, help the patient reschedule, update the original appointment, and deal with the new opening in the schedule. If nobody catches the response quickly, what could have been a simple reschedule can turn into a last-minute cancellation or missed appointment.
Henry Schein One’s benchmark data found an average 12% cancellation rate, a 4% no-show rate, and a confirmation rate of 44%. For a practice with roughly 36 appointments per day, or about 720 per month, that 12% cancellation rate represents around 86 appointments each month requiring some kind of schedule action.
Dental workflow estimates put routine confirmation calls at two to three minutes and rescheduling an appointment at three to five minutes. Using the low end of both assumptions, Annie can save practices an average of 28 hours per month on the basic mechanics of confirmations and rescheduling alone.
And again, the important word is basic. None of this is especially difficult work. But a missed confirmation response can quickly become an empty chair and a scramble to fill the schedule. A digital co-worker can catch the response, keep the conversation moving, and help solve the problem while it is small.
The surprising economics of very small tasks
Some of the most revealing opportunities are much smaller. Website chat is a good example.
Across practices using Annie, the average office receives about 16 website chats per month. The volume is almost comically low, roughly one conversation every other day. That makes it difficult to justify dedicating meaningful staff capacity to watching a chat window. Yet Annie says those same practices schedule an average of two to three appointments each month from those conversations.
That is what changes when the marginal cost of monitoring an opportunity approaches zero. A task does not have to happen frequently enough to justify staffing around it. It simply has to be valuable when it happens.
The same logic can apply to gathering missing insurance information from a patient, following up after someone leaves without scheduling, responding to an unusual after-hours inquiry, or triggering outreach when a relevant scheduling opportunity appears.
Individually, each workflow may be too small to earn a line item in a staffing plan. Collectively, they represent a surprising amount of unfinished work and lost opportunity.
The theme
The vision painted here is less dramatic than the robots and sweeping transformation often promised in AI hype. But it is also more practical. A dental practice does not need science fiction. It needs help making sure important, everyday work gets done consistently.
That is where the idea of a digital co-worker starts to make sense. It is not about one spectacular capability. It is about having a sidekick inside the practice that can keep an eye on hundreds of small things at once. Who became overdue today? Who left without reappointing? Which patient still needs insurance verified? Who responded to a confirmation text with a problem? Which prospective patient just showed up on the website with a question?
In a way, it is like hiring someone to help “babysit” every patient in the practice -- not in the sense of taking over the relationship but of constantly watching the administrative details around it. The digital co-worker can notice when something needs to happen, take care of the routine work, and pull the team in when the situation calls for judgment, context, or a human touch.
That may be one of the healthiest ways to think about AI in dentistry. The goal does not have to be transforming the practice overnight. It can be raising the baseline. Recare gets worked. Confirmation responses get handled. Insurance gets a first pass. Small revenue opportunities get followed up on. Fewer important things depend on someone remembering them at exactly the right moment.
Unlike many of the grander promises surrounding AI, this is not a vision that requires waiting for some distant future. Practices can begin working this way now. Annie’s new digital co-worker is one early example of what that model can look like, and it points toward a broader shift in how software may fit into a dental team.
For practices thinking about their next hire, it raises an interesting possibility. Some of the help they need may come from a new kind of co-worker, one that works alongside the team, keeps an eye on every patient, and quietly helps ensure more of the important work gets done.
